*I acknowledge that I am voluntarily participating in the Columbus Wizards 2025 Tryouts at my own risk. I release and hold harmless the Columbus Wizards, its staff, and affiliates from any and all liability for injuries, illness, or damages that may occur.

I understand that the tryout fee is non-refundable, participation does not guarantee selection, and I grant permission for my image to be used for promotional purposes. I have read and agree to the full waiver and terms of participation.
